DEATH OF A CITIZEN OF ARMENIA 

 

Information Sheet: 

 

Death of a citizen of the Republic of Armenia in the territory of the United States of America must be immediately reported to the Embassy of the Republic of Armenia in Washington, DC or the Consulate General in Los Angeles. If, according to the will of the deceased or the will of the next-of-kin, the dead body or cremated remains should be transported to the Republic of Armenia, the Embassy or the Consulate General issue an Armenian Death Certificate and a written statement confirming that the body in the casket or the cremated remains are that of the citizen of the Republic of Armenia, and that the casket or the urn does not contain any foreign object and is not a source of contagious diseases.

 

To apply for the above-mentioned documents, the relatives or next-of-kin of the deceased person, or third parties need to submit certain documentation to the Embassy or the Consulate General of Armenia, depending on the state where the death has occurred (see table below):

The following documents need to be submitted, by any registered mail with tracking number:

 

1. A letter from the relatives of the deceased or next-of-kin, or a third party requesting the issuance of the Death Certificate and the Statement.

 

2. Documents confirming the identity and Armenian citizenship of the deceased person (i.e., an Armenian passport).

 

3. An original notarized medical report on the cause of death.  

 

4. A written statement confirming that the body is embalmed or cremated, issued and signed by the person who finally seals the casket or the urn. The statement should also confirm that the casket or the urn does not contain any foreign object (drugs, weapons or other illicit materials) and is not a source of contagious diseases.

 

5. Please include in your original shipment a prepaid preaddressed return envelope of any registered mail with a tracking number or a money order or check payable to the Embassy or Consulate General of Armenia in the amount of US$20.00, if overnight FedEx™Express service is requested.

 

 

 

Notes:

 

1. Death Certificate and the Statement are issued by the Embassy or the Consulate General within 1-3 business days.

 

2. The above documents are issued free of charge.

 

3. It is necessary for the casket to be of zinc or stainless steel in a wooden container, which should have the following maximum dimensions:

 

- Length                       2.08m (81.95in)

- Width                         0.76m (29.95in)

- Height                        0.70m (27.58in)

- Weight                       150kg (330.75lb)

 

4. If an original American Death Certificate, legalized with an APOSTILLE (simplified certification by the Secretary of State in state of death, according to the provisions of the Hague Convention of October 5, 1961) is submitted, no Armenian Death Certificate will be issued.
